Our Team

Charlotte Brolsma

Charlotte is the founder of Form. and is a New York licensed Physical Therapist, Australian trained Sports Physiotherapist and Clinical Pilates Instructor. She is passionate about health and wellness and uses her skills and training to help you feel better and stronger - no matter what your goals may be.

Charlotte started her journey into sports, health and wellness during her teenage years where she competed in Gymnastics, Rowing and Athletics, and built on this with a career in Sports Physiotherapy and Clinical Pilates in Melbourne, Australia. Throughout her career she has worked with many different athletes from professional Australian Rules Football players, Rugby Union players, Long distance runners, Triathletes, gymnasts and with international elite athletes competing at the Commonwealth Games.

Charlotte has pilates training through The Australian Physiotherapy & Pilates Institute, a Bachelors degree with Honours in Physiotherapy from the University of Melbourne and a Masters in Sports Physiotherapy from La Trobe Univeristy.

Charlotte has now brought these skills to New York City where she educates and empowers her clients to be the best version of themselves through evidence based Physical Therapy, training and Pilates.

She uses her expert skills and attention to detail to examine the physical body and identify imbalances and limitations that may be holding them back from optimal performance in life and exercise.

Charlotte teaches a modern, technical and athletic type of pilates and clinical exercise to address these insufficiencies and to help build you up to be your best self, achieve your health and wellness goals and come back better than ever.

 

Maica Padios

Maica is a New York licensed Physical Therapist and a Classical and Contemporary Clinical Pilates Instructor. She has extensive experience diagnosing and treating musculoskeletal injuries, pre and post-surgical rehab, and neurological conditions. Maica uses the most current evidence based practice in order to help you achieve optimum functionality in the best possible way.

Denzel Cherry

Denzel holds a doctorate in Physical Therapy and is a New York licensed Physical Therapist. Denzel has worked with athletes at the high school, collegiate and professional level. He enjoys incorporating functional movement analysis and strength training principles to unlock human performance. Denzel's passion lies in helping individuals get back to what they love doing.

By the time I was under the care of Charlotte, I had already seen two doctors, a chiropractor, and a body worker, none of whom provided the elevated level of attention, knowledge, or capability to coach me through a torn labrum in my hip joint.
