In some cases, Physical Therapy can be partially reimbursable by your insurance company.
Form. does not participate as an in-network provider with any insurance providers at this time.
If you have insurance and are interested in possible reimbursement for Physical Therapy sessions you can ask your insurance company the following:
Do I have out-of-network Physical Therapy coverage?
Do I need a referral for access to my coverage?
What is my benefit rate and how much will I be reimbursed?
Is there a cap on the number of sessions?
Depending on your policy we can assist with submitting a claim or we can provide you with a receipt for out-of-network reimbursement. Our team will discuss your individual situation with you.
